Welcome New Patients
New patients are welcome. All new patients are asked to complete the patient registration forms online. Print the forms and email the completed forms along with a copy/photo of your Insurance card to info@thefootcenter.com.
At the time of the initial visit, the patient must present their insurance card, photo ID, and a credit card. This will make the registration process smoother. Prompt arrival is most appreciated.
Bring the completed forms to the office on your initial visit.
